How to appeal an Anthem claim denial
Anthem gives members 180 days from the denial notice to file an appeal or grievance. Anthem (part of Elevance Health) is a Blue Cross Blue Shield licensee, so the process mirrors other Blue Cross plans.

Why it's worth appealing
Across ACA Marketplace insurers, about 19% of in-network claims were denied in 2024, yet patients appealed fewer than 1% of those denials. When people did appeal, roughly one in three internal appeals ended with the denial overturned.
Step 1: Read your denial letter and EOB
Find the claim number, your member ID, the exact reason for the denial, and the appeal deadline. Common reasons include "not medically necessary," "out of network," "no prior authorization," or a provider coding error.
Step 2: Know your deadline (usually 180 days)
For most plans you have 180 days from the date you receive the denial notice to file an appeal or grievance. Medicare and Medicaid plans follow their own timelines — check your denial letter.
Step 3: Build your case
Request the records and plan criteria Anthem used to decide your claim, and get a letter of medical necessity from your doctor explaining your diagnosis, what was tried before, and why the care was needed.
Step 4: File the appeal with Anthem
- Sign in at anthem.com and go to Claims > Appeal a claim, or use the Sydney Health app.
- Or fax your written appeal to the number on your denial letter (keep a certified-mail copy as backup).
Anthem acknowledges appeals in writing within 5 calendar days, and must respond within 72 hours for an urgent (expedited) review. Keep copies of everything you send.
Step 5: Request an external review if denied
If the internal appeal is denied, you can request an independent external review, generally within 4 months of the final denial. An outside reviewer decides and Anthem must accept the result. The federal external review process is free.
