How to appeal a Humana denial
Most Humana members are in Medicare Advantage plans. You have 65 days from the denial notice to ask Humana for a reconsideration. If Humana upholds the denial, your case is automatically forwarded to an independent review entity — you don't have to re-file.

First, which Humana plan do you have?
This guide covers Humana Medicare Advantage (Part C), which is most Humana members. If you have a commercial Humana plan through an employer or the Marketplace, you generally have 180 days to appeal under the federal process — see our general appeal guide.
Step 1: Read your denial notice
Find the claim or reference number, your member ID, the exact reason for the denial, and the appeal deadline. Keep the envelope — your 65-day deadline runs from the date on the notice.
Step 2: Know your deadline (65 days for Medicare Advantage)
File a reconsideration with Humana within 65 days of the initial denial notice. If you miss it, you may still file late with a good reason.
Step 3: Build your case
Write a short letter explaining why the care should be covered, and attach a letter of medical necessity from your doctor plus relevant records and the denial notice.
Step 4: File the reconsideration with Humana
- Use the Humana Appeal, Complaint or Grievance form at Humana.com/Appeals, or
- Call the Member Services number on your Humana ID card for instructions.
Humana generally decides within 30 days for care you haven't received yet, 60 days for payment, and within 72 hours for a fast (expedited) appeal if waiting could harm your health.
Step 5: The appeal continues automatically
In Medicare Advantage, if Humana upholds the denial, it must send your case to an independent review entity for a fresh look — automatically. Beyond that, further levels exist (including a hearing before an administrative law judge) if the amount in dispute is high enough.
