How to appeal a UnitedHealthcare claim denial

UnitedHealthcare says you usually have 180 days from your first denial notice to file an internal appeal. If it's denied again, federal rules let you ask for an independent external review within 4 months — and the insurer must follow that reviewer's decision.

Why it's worth appealing

Across ACA Marketplace insurers, about 19% of in-network claims were denied in 2024, yet patients appealed fewer than 1% of those denials. When people did appeal, roughly one in three internal appeals ended with the denial overturned.

Step 1: Read your denial letter and EOB

Find the claim number, your member ID, the exact reason for the denial, and the appeal deadline. UnitedHealthcare groups most denials into five reasons: eligibility, duplicate submissions, missing documentation (like records or imaging), a service the plan doesn't cover, or care that didn't match its clinical guidelines.

Step 2: Know your deadline (usually 180 days)

UnitedHealthcare states that for most plans you have 180 days from the date of the first denial notice to appeal. Some plans allow more time — check your Summary Plan Description or Certificate of Coverage to be sure.

Step 3: Build your case

You can ask UnitedHealthcare, free of charge, for the documents used to decide your claim, including the specific plan rule or clinical guideline. Then get a letter of medical necessity from your treating doctor explaining your diagnosis, what was tried before, and why this care was needed.

Step 4: File the internal appeal with UnitedHealthcare

There are several ways to start:

Put the appeal in writing, attach the denial notice, your doctor's letter, and relevant records, keep copies, and send it in a way you can track. UnitedHealthcare generally decides within 30 days for care you haven't received yet and 60 days for care you already got. If waiting could seriously harm your health, ask for an expedited appeal.

Step 5: Request an external review if denied again

If the internal appeal is denied, you can request an independent external review, generally within 4 months of the final denial. An outside reviewer decides — usually within 45 days, or 72 hours for urgent cases — and UnitedHealthcare must accept the result. The federal external review process is free.
